UPPSC LT Grade Assistant Teacher DV Schedule 2026 OUT Check DV Schedule PDF
Uttar Pradesh Public Service Commission (UPPSC), Prayagraj has released the Document Verification (DV) schedule for provisionally successful candidates in the Assistant Teacher, Trained Graduate (LT) Grade (Male/Female Branch) Exam-2025. The DV process for Science, Commerce, and Agriculture/Horticulture subjects will be conducted on 25 and 26 September 2026 at Saraswati Bhawan (Ground Floor), UPPSC Office, Prayagraj. Candidates can download their DV form set starting 23 September 2026 via OTR login.

How to Download the UPPSC LT Grade DV Form Set 2026
- Starting from 23 September 2026, candidates can access and print their complete DV form set online.
- Visit the official UPPSC website at uppsc.up.nic.in.
- On the Home Page, find and click the link titled “Click here for Advt. No. A-5/E-1/2025, Assistant Teacher, Trained Graduate Grade (Male/Female Branch) Exam-2025 Document Verification”.
- Click on “Click here to Authenticate” when prompted.
- Authenticate your profile using your OTR number and password/OTP.
- After successful authentication, your “Applicant Dashboard” will open automatically.
- Click on “Check Latest Status” against the post you applied for.
- Click on “Proceed to Print Complete Form Set for Document Verification”.
- The online form set will be displayed (which includes the application form, certification form/प्रमाणीकरण प्रपत्र, declaration form/देशना-पत्रक, and marksheet details/अंकतालिका विवरण).
- Print the complete set, sign it where required, and carry it along with your original documents to the DV venue.
