- Maximum Age: 42 Years
- SC, ST, SEBC, and all Women candidates get 5 years of relaxation in the upper age limit.
- PwD candidates get 10 years of upper age relaxation, with cumulative relaxation up to 15 years for SC/ST/SEBC PwD.
- For Contractual Ophthalmic Assistants under OSH&FW Society, applications are allowed up to the age of 45 Years.

- Candidates must have passed +2 Science (12th in Science stream) or an equivalent qualification from a Council/Board that is recognized.
- Candidates must have passed the 2 Years Certified Ophthalmic Assistant (COA) Training Course OR the Certified Ophthalmic Surgical Assistant (COSA) Course. The institute must be Govt. or recognized by Govt.
- Registration with the Ophthalmic Officers’ Council in Odisha State is required. Candidates must also hold a valid Registration Certificate.
