How to Apply Offline for East Godavari ASHA Recruitment 2026
- Eligible female candidates must apply through Offline Mode.
- Obtain or download the prescribed ASHA application form from the official website or the local Primary Health Centre (PHC).
- Enter your details carefully: Candidate’s name, husband’s name, date of birth, social category, full communication address, and village/ward details.
- Mention the specific PHC or UPHC (Urban Primary Health Centre) where the application is being submitted.
- Provide your Aadhaar number, active mobile number, and email address.
- Attach self-attested copies of required certificates (Class 10th marks memo, caste, age proof, local residence proof) and the VHNC/VHSNC resolution document.
- Sign the declaration in the application form.
- Get the application forwarded by the Chairman of the respective VHNSC/UHNSC (Village/Urban Health Sanitation and Nutrition Committee), where applicable.
