• The candidate must currently be a Contractual Employee under NHM and OSH&FW Society, holding the same post in another district of Odisha.
• The educational qualifications and valid registration required for the post must be held as specified by NHM norms.
• The required tenure must be completed with satisfactory performance. A valid NOC issued by the appointing authority is also required.
• Only existing contractual employees under NHM can apply; applications from outside candidates will be rejected.

2. Complete the application form carefully using block letters. Fill in Post Applied For, Personal Details, Present Contact Details, Employment Record, Place of Posting, and Renewal Details.
3. Attach self-attested photocopies of all mandatory documents. Include all certificates and mark sheets supporting educational qualifications, along with 02 recent passport-size colour photographs that are self-attested. Add a self-attested photocopy of Photo Identity Proof (Aadhaar / Voter ID / PAN Card). Include the Original Appointment Order and Joining Report for the same post, plus transfer and joining orders, if any. Attach the No Objection Certificate (NOC) cum Experience Certificate issued by the concerned CDM&PHO / appointing authority. Provide contract renewal proof through the latest renewal order / copy of agreement.
